This session will demonstrate how a population health approach was taken for the ‘Innovation for Healthcare Inequalities Programme’ in North-East London. This includes how targeted hypertension case finding was conducting in the community, for primary prevention, linking into community pharmacy services. Alongside this, GP registers were reviewed for opportunities to optimise innovative lipid lowering therapy for secondary prevention. | ||||
