This session will share the experience of implementing region-wide DOAC medicine optimisation reviews across Primary Care with the aim to improve quality of prescribing and safety for patients with atrial fibrillation. In addition, clinical support, education and training was provided to facilitate safe switching of patients to the best value DOAC as per NHSE recommendations in a safe and consistent way. This session will share the lessons learned including the enablers and barriers to delivering such a project. | ||||
