This session will discuss the vision and strategy which has lead to an established award-winning pharmacy team run from Barts Heart Centre focused on cardiovascular prevention through collaborative working with primary care across NE London including practice-based pharmacists and community pharmacy. Within the session, we will discuss some of the innovative initiatives that have successfully transformed patient care for people at high risk or living with established cardiovascular disease, highlighting the importance of relationships and collaborative working. | ||||
