This session will highlight the British Society for Heart Failure’s 25-in-25 initiative which aims to reduce the mortality from heart failure in the first year after diagnosis by 25% in the next 25 years. This would mean 5 fewer deaths for every 100 patients newly diagnosed with heart failure every year, translating to over 10,000 lives saved per year. We will discuss the implementation through a blueprint for local health economies, co-designed by patients and heart failure stakeholders, with practical and achievable solutions to tackle this huge problem. |
