This session will cover:
Heather has worked as a Consultant Pharmacist at Leeds Teaching Hospitals NHS Trust since January 2013 and works for NHS Leeds CCG 1 day per week reviewing complex patients and supporting primary care pharmacists. Heather leads on improving medicines optimisation for older people, including improving medicines communication and handover for older people being discharged from the Trust and leading the Connect with Pharmacy project for referring patients to Community Pharmacy. Heather has also been involved in a number of continuous improvement work streams to reduce falls. Heather’s research work includes deprescribing and jointly supervising a PhD student with the University of Bradford researching medicines pathways for frail older people with another joint PhD in pharmacogenomics in older people starting soon. She has a keen interest in education and training and is a fellow of the Faculty of the Royal Pharmaceutical Society. Foundation Level. |
