Reducing the risks of medicines related hospital admissions for older people living with severe frailty. People living with severe frailty are more likely to take medicines and suffer with medicines related adverse events, including unnecessary hospital admissions and poor patient orientated outcomes. In hospital, they have higher mortality, increased length of stay and increased risk of drug related readmissions post discharge. The session will demonstrate how pharmacists in various care settings can reduce the risks of these adverse events by leading the delivery of medicines optimisation within integrated care teams for older people.
