Care Home Pharmacist/Medicines Project Lead, Croydon CCG / Guy's and St Thomas' NHS Foundation Trust, Integrated Care Transformation Programme
Cherise Gyimah has over 15 years’ experience working in primary and community settings. As a Care Homes Pharmacist in Croydon, she leads on medicines optimisation and clinical governance issues relating to care homes. This includes liaising with relevant staff to review current practice and establish safe and effective arrangements for managing medicines. She is keen to enhance the care of people in care homes, supporting them to maintain their independence as far as possible.
Cherise also works with Guy’s and St Thomas’ Adult Local Services, leading on a project to deliver a person-centred and integrated approach to medicines support for adults in the community.
She is a NICE Medicines and Prescribing Associate, member of the Q community, London Lead for the Primary Care Pharmacy Association Care Homes Group, Specialist Advisor with the Care Quality Commission and Co - Chair of the English Deprescribing Network (EDeN).
